FDA Actions: The Compounding Pharmacies We Name
We tell readers that a provider naming its compounding pharmacy is a good sign. That only helps if someone then checks the name. So we did — searching FDA's public records for every compounding pharmacy named anywhere on this site, including the ones our own top-ranked providers use.
Read this before the table
A Form 483 is not a finding of violation. The FDA describes it as a list of observations made by investigators during an inspection. Firms respond to them and routinely resolve them.
A Warning Letter is more serious — a formal notice of alleged violations — but it is still a point-in-time record. The FDA operates a close-out letter process for firms that correct the issues, and several records below are years old.
None of this tells you what a pharmacy's practices are today. It tells you what is in the public record and where to read it. That is genuinely useful, and it is all we are claiming.

Context: The August 2025 Form 483 is recent, but a 483 is an observation list issued at the end of an inspection — not an enforcement action and not a finding that a violation occurred. Firms respond to 483s and frequently resolve them.

We searched FDA's published warning letters for each of these and found none. That is a negative search result, not a certification — absence of a warning letter is not the same as an FDA endorsement, and our search was not exhaustive.

What this changes about our rankings
Honestly: less than you might expect, but it is worth stating plainly rather than burying.
Polly ranks highly with us partly because it names its compounding pharmacies, one of which is Hallandale — and Hallandale has a 2020 FDA warning letter citing serious deficiencies in sterile drug production. We are not withdrawing that ranking, because the alternative providers mostly decline to name any pharmacy at all, and you cannot check what you are not told. But a reader deserves both facts, not just the flattering one.
The same applies to TrimRx, which names Olympia Pharmaceuticals — a firm that received a Form 483 in August 2025.
The uncomfortable implication. Transparency exposes a provider to exactly this kind of scrutiny, while a competitor that says nothing appears clean. If naming a pharmacy becomes a liability, providers stop naming them — which is worse for patients. Weigh a disclosed, checkable record against an undisclosed, uncheckable one accordingly.
How to check a pharmacy yourself
- Ask your provider which pharmacy compounds your medication, and whether it is a 503A pharmacy or a 503B outsourcing facility. If they will not say, that is your answer about transparency.
- Search the FDA Warning Letter database for the pharmacy's name and any legal entity name it trades under — the two often differ, as with Pharmcore and Hallandale.
- Check FDA's compounding inspections, recalls and other actions page, which lists 483s and recalls alongside warning letters.
- Verify the pharmacy's licence with the relevant state board of pharmacy.
- Read any record in full and note the date. A six-year-old letter and a recent one are not the same signal.
